This form is designed to help parents and caregivers check their child’s development across a range of areas including communication, social interaction, play, movement, learning and everyday skills.
The questions are based on developmental milestones and information from the Learn the Signs. Act Early. program developed by the Centers for Disease Control and Prevention (CDC).
This questionnaire is a screening tool only. It is not a diagnostic assessment and cannot determine whether a child has a developmental delay, disability or diagnosis.
After you complete the form below, we will be in touch to schedule an appointment to review the results.
Children develop at different rates, and many factors can influence how and when skills emerge. The purpose of this questionnaire is to identify whether your child may benefit from further exploration, monitoring or support.
If the questionnaire identifies possible areas of concern, this does not necessarily mean that something is wrong. It simply suggests that discussing your child’s development with a health professional (such as your GP, child and family health nurse, paediatrician, psychologist, occupational therapist or speech pathologist) may be helpful.
We encourage you to answer the questions as honestly as possible based on what you typically observe in your child’s everyday environments.
